GREENDALE TWI NITE
PLAYER REGISTRATION
GENERAL TERMS & CONDITIONS
- I/We, the parents/guardians of the above-named candidate for a position on a Greendale Twi Nite team, hereby give my/our approval to participate in any and all Greendale Twi Nite activities, including transportation to and from the activities.
- I/We know that participation in baseball or softball may result in serious injuries and protective equipment does not prevent all injuries to players, and do hereby waive, release, absolve, indemnify, and agree to hold harmless the local Greendale Twi Nite, Greendale Twi Nite Baseball, Incorporated, the organizers, sponsors, supervisors, participants, and persons transporting my/our child to and from activities from any claim arising out of any injury to my/our child whether the result of negligence or for any other cause.
- I/We agree to return upon request any equipment or items issued to my/our child in as good conditions as when received except for normal wear and tear.
- I/We agree to provide proof of age (as defined by Greendale Twi Nite Baseball, Incorporated) upon request. I/We understand that our child (candidate) must be eligible under the age regulations of Greendale Twi Nite, Incorporated, to participate in this League, and that if any controversy arises regarding residence and/or age, the decision of the Greendale Twi Nite Board of Directors shall be final and binding. I/We further understand that if any participant on a Greendale Twi Nite team does not qualify for participation in the league based on age (as defined by Greendale Twi Nite Baseball, Incorporated), such participant and/or team on which he/she participates may be found ineligible, and forfeit(s) and/or suspend League or Tournament privileges.
- I/We understand that our child (candidate) may be selected to play on any Greendale Twi Nite team including a Major Division team. Team selection and placement decisions are solely at the discretion of Greendale Twi Nite. Individual players may request specific team placement, however such requests do not guarantee or obligate Greendale Twi Nite in any way. Refusal of a player to participate on a particular team may result in suspension from play without refund of player registration fees and/or further restrictions by Greendale Twi Nite.
- I/We will furnish a certified birth certificate of the above-named candidate to Greendale Twi Nite Officials upon request.
- I/We agree and consent to the use of my/our photo, video or film and or my child(ren)’s photo, video or film publicly in association with efforts to promote Greendale Twi Nite, player registration, league sponsors and or other marketing efforts. I understand images or videos may be used online, in print, during presentations or on web and or social media. I further understand no royalty or fee or other compensation will be provided to me or payable due to such use.
- I/We agree and understand players may be suspended, banned or terminated from team and or league participation based on behavior. Such suspensions, bans or removal are at the sole discretion of Greendale Twi Nite. Further parents, guardians and or siblings similarly demonstrated inappropriate behavior or conduct whether during games, practice or other events may similarly be removed or prohibited from future participation. Any such removal, suspension or termination of player rights due to behavioral matters will result in a forfeiture of all player fees. No refund will be provided.
